What Is a Follicular Study?
A follicular study, also known as folliculometry, is a series of ultrasound scans performed to closely monitor the growth and development of follicles during a menstrual cycle.
The follicles are monitored from the early phase of the cycle until a dominant follicle matures. Once the dominant follicle reaches maturity, an HCG (Human Chorionic Gonadotropin) injection may be administered to trigger ovulation.

How Is a Follicular Study Performed?
A follicular study involves a series of ultrasound scans, usually performed between day 8 and day 20 of the menstrual cycle. It is typically done using a transvaginal method. A thin probe is covered with a protective cover, and a layer of lubricant gel is applied.
The patient is asked to lie down on the examination table with legs slightly apart. The probe is gently inserted into the vagina, and clear images of the ovaries and follicles are displayed on the monitor.
The ovaries are examined to monitor follicular growth. One follicle grows faster than the others and becomes the dominant follicle, which will later rupture.
The dominant follicle is closely monitored in each scan until it reaches approximately 20 mm in size. An HCG injection is given once the dominant follicle reaches its ideal size.
Ovulation typically occurs within 24–48 hours after the HCG injection. The couple is advised to have intercourse during this time frame to increase their chances of conception.

Interpretation of Follicular Study Results
A follicular study is usually performed between day 8 and day 20 of the menstrual cycle, depending on follicular growth.
- The number of scans may vary between 4 and 6, based on follicular development.
- The ovaries initially contain multiple small follicles (MSF).
- One follicle becomes dominant and continues to grow.
- Once the dominant follicle matures to around 20 mm, an HCG injection is given to induce ovulation.

Follicular Study vs Ovulation Prediction Kits (OPK)
Both follicular studies and ovulation prediction kits are used to predict ovulation. However, a follicular study is more accurate because the dominant follicle is monitored directly through ultrasound.
Ovulation prediction kits are urine-based tests that detect a surge in LH (Luteinizing Hormone). A darker test line indicates rising LH levels, suggesting ovulation is approaching.
